9 months of age

no immunizations (unless needs flu vaccine*)

12 months of age

immunizations:  Proquad (MMR/Varivax), Prevnar 13, flu vaccine* - (if hasn't received earlier)

immunizations:  Hiberix, Hep A, flu vaccine* (annually)

18 months of age

M-CHAT questionnaire
immunizations:  DTaP, flu vaccine* (if hasn't received that year)

immunizations:  Kinrix (DTaP/IPV), Proquad (MMR/Varivax), flu vaccine* (if in season)

5 years of age

immunizations:  Catch up on Kindergarten vaccines if needed, flu vaccine* (if in season)

 

* Please note:    *The influenza vaccine (flu vaccine) is given annually, starting at 6 months of age.  The first year your child receives the flu vaccine, in order to be optimally protected, he or she requires 2 doses given 1 month apart.  Thereafter, your child will need one dose yearly. Flu vaccines are usually available annually between August and February each year, while supplies last.
